GENERAL INFORMATION:

The City of Chino Hills (“City”) invites your organization to submit a written proposal to provide on-call concrete services. The successful proposer will be awarded a maintenance contract for a five-year period. The proposal must clearly demonstrate how your organization would best satisfy the requirements of the City. This written Request for Proposal (RFP) states the scope of the City’s requirements and specifies the general rules for preparing the proposal.

Qualifications:
The successful contractor shall maintain a Class A, B or C-8 throughout the terms of the agreement and abide by the latest edition of the MUTCD Standards for Temporary Traffic Control Zones (TTC Zones) while performing work in the City of Chino Hills.
Special Requirements:
The proposer must submit a package clearly marked on the outside as “Proposal for On-Call Concrete Services” to the City Clerk’s Office. The package must contain the following:
• Three (3) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal;
• One (1) original and two (2) copies of the cost proposal in a separate sealed envelope clearly marked “Sealed Cost Proposal”; and
• A signed W-9.
