The following is a summary only and does not replace the full RFP.
 
Background:

The City of Chino Hills is seeking a qualified firm to perform engineering design services associated with the Ramona Avenue Water Main Replacement Project.  The project will replace approximately 700 linear feet (LF) of 2-inch water line, water services and saddles, fire hydrants and other necessary appurtenances.  

In addition, the project will include engineering design of concrete sidewalk, driveway aprons and access ramps, identify recommended improvements for the adjacent private property on the east side, and pavement resurfacing/rehabilitation of existing asphalt pavement area on Ramona Avenue between Mesa Boulevard and Bird Farm Road.

The submittal package shall include: five (5) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal, one (1) original and four (4) copies of the cost proposal in a separate sealed envelope clearly marked "Sealed Cost Proposal" and a signed W-9.

Proposer must be willing to enter into an agreement with the City of Chino Hills. A copy of the agreement accompanies the full RFP. Please review this agreement and provide the City with a written statement of your firm's willingness to accept the terms of the agreement. Please specifically identify each and every term of the agreement which your firm is unwilling to accept and the reason therefore.
