The City of Chino Hills is a Passport Acceptance Agency.
Requirements
All applicants are asked to bring the following items:
1. New Passport Application Completed Form DS-11
or
Renewal Application Completed Form DS-82
2. Submit Evidence of Citizenship-
*Certified birth certificate issued by the county or state
Consular Report of Birth Abroad
Naturalization Certificate
Certificate of Citizenship
3. Present Identification-
Current Driver's License
Current Government ID (city,state,or federal)
Current Military ID
4. Provide Two Passport Photos-
Photos can be taken in the City Clerks Office for $9.60
Quality requirements for Passport Book & Passport Card photographs
5. Two separate checks or money orders are required
6. A copy of I.D., both front and back (2 sided copies will not be accepted)

*A certified birth certificate has a raised embossed, impressed or multicolored seal. The registar's signature and date must be visable. Abstract versions of birth certificates may not be acceptable for passort purposes.
*Children under the age of 16, must be accompanied by both parents.
